You can thank your Misericordia caregiver today! The Grateful Patients program provides patients and their families with an opportunity to show gratitude to those who played a special role in the care they received while at MHC.
Your support will not only recognize a compassionate caregiver, but will also help MHC deliver the Future of Care – excellence in eye care, treatment for chronic conditions, and a continuum of care for older adults - to all Manitobans. MHC is committed to providing exceptional care to all. Whether you choose to acknowledge a special nurse, doctor, or an entire department, your donation is a meaningful opportunity to say thank you. |

PRIVACY DISCLAIMER:
In 2008, the Personal Health Information Act (PHIA) was changed to allow health-care based fundraising organizations to contact past patients. As such, Misericordia Health Centre Foundation (MHCF) uses the information provided by Misericordia Health Centre (MHC) to run the Grateful Patient (GP) program, allowing former patients to express gratitude for the care they received at MHC. With this program, grateful patients and their families can send a personalized message to their caregiver and make a donation to support MHC.
Data files are received from the Health Information Services Office (HISO) no more than once per quarter. Each file is representative of patients who used MHC services three months back, which allows an adequate response period for each patient to opt-out. Deceased patients, those who are undertaking lawsuits, have filed complaints, and minors are removed from the list. If a patient has been admitted multiple times in the month, their name will occur in the file multiple times, but they will only be mailed to once. Only names and addresses are provided by MHC to MHCF, no additional information is disclosed (such as reason for visit, PHIN number, or personal health information).
Patients can opt-out from having their information provided to MHCF upon filling out the patient registration forms. If the opt-out was missed and a mailing was received, patients still have the ability to be removed from future mailings.
Personal information is kept in confidence and only used to keep you informed about our activities, special events, funding needs, and more. The Foundation does not rent, sell or trade any personal information with third parties. If at any time you have questions regarding your information on file with us, or wish to be removed from our list, contact us by phone at 204.788.8458 or via email at [email protected].
